Dr’s Aaron Brennan, Aileen Faherty and John Morris left Briarhill Family Practice at the beginning of March 2018. Their departure resulted in Briarhill Family Practice having to close its doors. As the GP’s left Briarhill Family Practice without notice, we were not in a position to let all patients know in a timely manner. We realise that this will have caused significant distress and confusion.
If you are in receipt of a Medical Card or Doctor Visit Card you should have already been informed of the above by your chosen doctor and all of your medical records have been securely transferred to your chosen doctor. If you have concerns or queries in relation to your medical card, medical records, your chosen GP information, etc. you can contact the HSE National Medical Card Unit on 1890 252 919.
IMPORTANT: It is intended that all private patient medical records will be transferred to Dr’s Brennan, Faherty & Morris. If you do not wish your records to be transferred, please email email@example.com before Tuesday April 3rd 2018.
If you are happy to have your records transferred to Dr’s Brennan, Faherty & Morris then you do not need to take any further action. If your GP requires your medical record urgently they can arrange this without any difficulty.
If you wish to transfer your records to a new GP, please email firstname.lastname@example.org so that signed consent to transfer your medical records can be organised over the coming days. A full list of GPs available in the Galway region may be found here – GPs in Galway.
